1. You MUST submit this form to receive a Return Merchandise Authorization number (RMA). A unique RMA number needs to be assigned to your product BEFORE we can accept your product for warranty, troubleshooting or repair. NO EXCEPTIONS. Repair fees will be charged to you unless product is still under warranty.
2. When sending your product to our service center, your RMA number MUST be written on the outside and inside of the box you are ship to us.
